Declaration of Major and Minor - Form and Instructions

To start the declaration process, view Declaration of Major & Minor Contacts and find the department you would like to declare a major or minor in. Email the person listed to ask what steps you need to follow to declare a major or minor. 

A major must be officially declared before a minor can be declared. If you are still listed as undeclared in SIS, you cannot declare a minor.

Review the preview form to see what information you'll need to complete BEFORE starting the DocuSign process (required fields are highlighted in yellow on the preview). Do not submit this preview form. Submit the DocuSign form.

When you have completed the above tasks, click on the Declaration of Major and Minor Form below and begin with the following steps:

1. Enter your name and UVA email into the Student section.
2. Enter the name and email of the Declaration of Major Contact for the department you are declaring a major or minor in.
3. Click on "Begin Signing" at the bottom of the form.
