Department Contact for Request for Transfer of Credit: Major or Minor

Want to apply transfer credit from a U.S. institution toward your declared major or minor in the College?

You must obtain approval in advance by submitting the Request for Transfer of Credit - Major or Minor Form through DocuSign. The form must be signed by the department before it is routed to the College Transfer Credit Evaluator for final approval. To route your form to the department for signature, enter the name and email of the Department Contact for your department from the list below in the Department Contact section of the PowerForm at the beginning of the DocuSign process.

Failure to route the form to the correct Department Contact may result in your form being rejected or voided as incomplete.

If you have not yet declared the major or minor, do not use this form. Submit your request using the Request for Transfer of Credit - Elective Form.
